How to do an online questionnaire?

we have got a project, where we need to do a questionnaire. The bases: the teachers need to log in to choose from 100 prewritten questions 10, than they give a name to it. After, they tell this name to the students who write this code to the website and they fill in the answers. Afterwards, the teachers can look at the answers, in lists and on diagrams. And the students need to be unidentifiable.
So, as you see, we don't get it, we would like to hire someone near, but we want to know what is the best platform for it, etc.

If you can't do it and you plan to hire someone to do it, let them worry about the tech stuff.
