transfer function of block diagram

I need advise.
using the state variables given in the block diagram; whats the state space model of the system given?what confuses me : in the figure two inputs are linked together, and there are two outputs( but one of the output is named as state variable x4 )
also can i get the transfer function of the attached block diagram in simulink? thanks in advance.
